Application for
Funding
The Colburn National
Foundation’s goal is to assist children with
disabilities up to and including age 21 and their families
through direct support or supporting 501(C) 3 organizations.
We are particularly interested in assisting young people
disabled by birth related traumas, neurological impairments,
musculoskeletal disorders, acquired brain injury, congenital
anomalies, and metabolic & growth disorders. The primary
geographic focus of the Foundation is within a 50-mile
radius of Saratoga Springs, New York, although the Board of
Directors may determine, from time to time, to assist
children, young adults, and their families or supporting
organizations outside of that area as need requires and
financial resources allow.
Grants are awarded on the basis of need. Applications
are reviewed upon arrival and funding decisions are made as
soon as possible after receipt. At times, it may be
necessary to place an approved request on a waiting list
until funds become available. Applicants applying for the
first-time will be funded on a first-come, first-served
basis. Grant recipients may reapply for additional funding
one-year after receiving funding.
Confidentiality/Privacy Statement:
The process of grant recipient selection requires the
sharing of sensitive medical and financial information. The
Colburn National Foundation assures you that this
information will be held in the strictest of confidence. The
information shared will be used only for the purpose of
selecting grant recipients. In addition, please complete the
attached Protected Health Information (PHI) authorization
for release.

Eligibility Requirements:
Grants to supporting organizations will only be made to
not-for-profit corporations that are registered 501(c)3
organizations.
Grants requested for individuals must be age 21 or
younger.
IMPORTANT:
Individuals applying for grants must have a
physician-documented disability which includes a statement
of why the grant requested will be beneficial to the
pediatric applicant. Original documentation with the
physician’s actual signature must be submitted; no
photocopied or stamped signatures by the physician will be
accepted. Medical documentation that is over one-year old is
not acceptable.
The liquid assets (cash,
checking and savings accounts, money market accounts,
stocks, certificates of deposit, bonds and mutual funds) of
the patient and family will be considered in determining
awards. Liquid assets do not include car or house.
Grants are not provided to individuals living in
residential care facilities.
